JOB SUMMARY:  Provides Case Management and Counseling to Adult mentally ill consumers.  Will work as part of a multi-disciplinary treatment team, providing case management services, primarily in community-based settings.  Works flexible hours and participates in 24-hour, 7-days per week emergency telephone services.
 
TRAINING AND EXPERIENCE REQUIRED: Bachelor degree in behavioral science discipline from an accredited institution.  State licensure or certification when applicable.  Ability to relate well to seriously mentally ill individuals and their families.
 
The Case Manager shall have access to reliable transportation and maintain liability insurance appropriate for use with this job including transportation of clients.  The Case Manager shall have or obtain, and thereafter maintain, a proper driver’s license to transport clients in their vehicle or a passenger van; shall maintain a safe driving record; and must be considered insurable through Fortwood Center’s insurance carrier.
 
The Case Manager shall also maintain a home telephone and accept business/client related calls as needed.
 
PHYSICAL & MENTAL REQUIREMENTS:  The duties associated with this job are performed in a variety of settings, including, but not limited to, a normal office environment, consumer residences, hospitals, clinics, retail establishments and outdoors.  Exposure to a variety of environmental factors including: temperature extremes, precipitation, dust, fumes, odors and vermin is frequent.  Physical requirements include the ability to physically assist consumers in and out of vehicles, up and down stairs and otherwise assist them in negotiating barriers to mobility.  Additional physical requirements include the ability to write or dictate progress notes, sit for long periods, walk, run, climb stairs, stoop and occasionally lift moderate to heavy loads of 75 lbs. or less.  Sensory requirements include normal or corrected vision to allow reading of handwritten and/or typed information, the ability to hear normal to soft tones, and to distinguish smells and temperature changes.  Use of equipment includes the operation of a motor vehicle, land line and cellular telephones, pagers, copy machine, fax machine, and filing cabinets.  Additional requirements include the ability to work under moderate to intense stress, the ability to relate well to mental health consumers and the ability to quickly escape dangerous or potentially dangerous situations while working alone with mental health consumers in a field environment.
